On 10 Oct 66, whilst in support of 5 RAR on Operation CANBERRA this aircraft was shot down and destroyed.
The pilot (2Lt Bill Davies) was seriously injured when hit by enemy fire and his passenger was injured in the subsequent crash landing.
The wreckage was recovered back to Vung Tau. |

"The pilot had shut down the helicopter to kill and load meat for a stock camp. After starting and completing the after-start checks, the pilot first entered a low hover. The aircraft was then put into a high hover about 10 ft above the tree tops before a climb was initiated into a wind of 15-20 kts. The pilot reported that as he accelerated through about 40 kts, he noticed a substantial reduction in the manifold pressure reading with the engine RPM at about 3000. The aircraft then began to sink into the tree tops. Some 100 metres further on, the main rotor blades struck a large tree trunk and the aircraft fell to the ground on its right side. The pilot was rendered unconscious at impact, but after regaining consciousness he assisted the surviving passenger from the wreckage and freed the other passenger's leg from under the cabin before dragging him clear. A strip inspection of the engine and turbocharger revealed significant defects. These included the density controller was set below tolerance; a cracked and warped wastegate butterfly; and a sticking wastegate actuator. Physical damage to the engine was attributed to poor engine handling. The cumulative effects of these defects would have reduced the available power output from the engine, but do not in themselves explain the symptoms reported by the pilot. In addition, the power available was sufficient for the pilot to execute a towering take-off. A turbocharger failure would have produced the loss of power experienced by the pilot, but no possible cause or result of such a failure was discovered. The pilot reportedly did not make a significant reduction in power after the high hover, so that it is unlikely that a sticking wastegate caused the problem. The cause of the reported loss of power was not determined." |

Pilot: 2Lt A D Jellie ASn39510*. When supporting the land clearing operations for Night Defence Position “Commanche”, when on touching down on the designated landing point, the front of the skids contacted solid ground but the rear of the skids continued to sink into 18 inches of mud.
At this stage, the tail rotor contacted the ground and was bent. The Transmission was replaced as was the tail rotor gear and extension boom.

161 Recce Flight Vietnam 1725hrs 04/08/1969 Horseshoe Feature (107.14E and 10.34N)Airframe Hours 270.25Hrs. Pilot: 2Lt G N Longland ASn1201446 commenced a take-off at “Horseshoe” from the recognised landing area( four pieces of PSP painted white and laid out in a square).
On application of pitch, the aircraft commenced to roll to the right. Pilot lowered the collective lever quickly which resulted in a heavy landing. At this stage, the aircraft engine was closed down and the pilot made a visual inspection on which it was noticed that the skid shoe securing bolt had caught the PSP. Damage to the aircraft was a bent cross tube and a cracked starboard rear upright tube on skid. A new rear leg assembly was transported to site by WO2 K Scott and fitted, with the a/c recovering to Nui Dat where a heavy landing check was initiated. The rear cross tube and saddle clamps were replaced.
161 Recce Flight Vietnam 1435hrs 16/08/1969 Luscombe AAF(107.14E and 10.34N)Airframe Hours 281.250Hrs.
Pilot: Capt F J Marcrow ASn235214 (Along with Sgt Jobling, Engine Fitter) on successful test flight, rejoined landing circuit when pilot noticed slight feedback to cyclic control stick and remarked to Sgt Jogling that he suspected a hydraulic malfunction which made him cut short the base leg. Ten seconds later the aircraft had a complete hydraulic failure. Because of the proximately to the ground, it was not possible to turn the hydraulic switch to the Off position. The cyclic stick was in a continuous "stirring" motion counterclockwise and he had little control over it. In order to get the aircraft down on the ground, it was necessary to wait until the a/c was approaching the skids level position and then push the collective lever down. A/c was landed without further incident.
Inspection of the aircraft immediately after return to Luscombe showed that the blanking plug on the hydraulic pump outlet had come loose, allowing the complete loss of pressure.
161 Recce Flight Vietnam 1545hrs 23/09/1969 nearby FSB Diggers Rest Bain Hoa Prov Sth Vietnam.(107.00E/10.43N). Airframe Hours 460.15Hrs.
Pilot:2Lt A G Kerr ASn217228 (With 2Lt Thomson in another a/c) carrying out a visual reconnaissance. 2Lt Thomson advised 2Lt Kerr that there was white smoke trailing from his aircraft.
A check of the cylinder head temperature gauge revealed that the reading was 290C degrees(246C degrees red line), transmission oil temperature 130C Degrees (130C Degrees Red Line) and Engine oil temperature 113C Degrees(Upper Red Line). Diverted to FSB Diggers Rest 3 minutes later. On landing, it was revealed that a fan belt thrown from the pully, with the second in place with excessive slack and oil leaks around engine. Inspection of the engine revealed a bent and broken push-rod housing tube and oil escaping from the fracture and burning on the hot exhaust pipe causing the white smoke during flight. The engine was changed as well as the transmission.

Pilot: 2Lt A D Jellie ASn39510*. On landing at refuelling point at FSB Diggers Rest, during heavy rain, the tail rotor struck the fuel filter at the POL point. An Anti-Torque failure landing was carried out and the aircraft shut down. The transmission and tail rotor gear box assembly were sent for repairs.
