The stdb engine failed at 100ft after take-off from Colomba Satellite airfield under control of the student pilot (75447 LAC JL Hoskin).  The Instructor pilot (408877 FLTLT EM Rosman) took control but the aircraft was too low and too slow to maintain flight.  Pilot was forced to make a wheels-up landing in a paddock adjacent to the satellite field.  Both aircrew received minor cuts and abrasion.  Moderate damage to fuselage and engines, both propellors damaged severely.  

Recommended for repair at unit.

Transferred (unrepaired) to reserve storage at 6SFTS.

Transferred to Mallala Station HQ for Category E storage.

Passed to DAP for disposal action

Sold by DAP at the Aircraft Remnants Auction RAAF Mallala to Mr. F.G. Conquest of Mitcham, Adelaide SA.  The aircraft was towed [sans wings] to Conquest House, Mitcham.  Aircraft was then sold to Mr. Tilley of Mitcham, Adelaide.  Nothing remained of the aircraft by 1965.

LAC ROBERTS, Vincent Clifford Hastings RAAF 121027 of 1 Service Flying Training School was killed when he was struck by propeller at Point Cook
Brakes locked after take-off.  Aircraft forced to land wheels up causing extensive damage.  Pilot 411333 FSGT RI Kesby and student 429149 LAC W.R. Elphanstone were uninjured.

While on a solo revision flight the aircraft was seen to enter a spin from which it did not recover.  The aircraft was observed to hit the ground three miles NW of the Little River Satellite Airfield near Point Cook.  The aircraft was severely damaged and recommended for conversion. The pilot 19288 LAC John Bernard Wallace of East Melbourne later died from his injuries at the RAAF Laverton Hospital, he is buried at the Springvale War Cemetery, Melbourne in Grave No 1.R.D.2

On landing finals 200 yards SW of Werribee Satellite Airfield at 1430 hrs, R6191 (Pilot LAC AE Southern) collided mid-air with Oxford AS687 (Pilot LAC Douglas Linden Bussenschutt).  416928 LAC Bussenschutt (28) single of Kadina, SA was killed and LAC Southern was severely injured.  Both aircraft were written off.

LAC Bussenschutt is buried in Kadina Cemetery, SA Protestant Block 74, Path 47.

X7139 took -off from Werribee Satellite Airfield at 0745 hrs in a northerly direction. The sole eyewitness stated that as the aircraft climbed to about 200ft it collided with Oxford R6194 and X7139 then immediately dived into the ground where it burst into flames and incinerated the two crew members. R6194 also lost height rapidly crashing through HT lines and trees on the NE boundary of the airfield before coming to rest in a paddock on the west side of Geelong Road.  R6194 was also completely destroyed in the accident; two of the three crewmen aboard were killed and 432525 LAC J.G. Legoe was seriously injured. Those killed in the accident were:

X7139 Pilot 408615 FLGOFF Alfred Jack Allen (23) married of Melbourne, VIC and Trainee 432063 LAC Dallas William Lynch (20) single of Krambach, NSW. 

R6194 Pilot 271521 FLTLT George Hector Howard (40) married of Murrumbeena, Melbourne and Trainee 20755 LAC Arthur Austin Hughes (32) married of Richmond, VIC

FLGOFF Allen was cremated at the Springvale Crematorium, Melbourne and is interred in Wall 2 Sec. AA Niche 519

LAC Lynch is buried in the Springvale War Cemetery, Melbourne. Grave 2.S.D.11

FLTLT Howard is buried in the Springvale War Cemetery, Melbourne. Grave 2.S.D.7

LAC Hughes is buried in the Geelong Eastern Cemetery Plot D. Row A Grave 5

Aircraft with five passengers and freight flew from RAAF Base Laverton to the Wooloomanata Satellite Airfield, VIC.  Landing at an unfamiliar airfield the pilot landed short onto rocky ground.  Despite applying full power and trying to perform a go around the aircraft heavily impacted the ground resulting in the undercarriage being ripped off and severe damage to the aircraft.  All aboard suffered minor cuts, bruises and some broken bones. Pilot 410053 FSGT R.V. Johnston subject to disciplinary action.

No 1 Central Recovery Depot at Werribee Satellite Airfield tasked to salvage the aircraft.

Aircraft recovered and converted to components by 1CRD staff.

At approximately 0915 hrs while flying a routine coastal training flight 18 miles out to sea the port engine failed, so the pilot headed back to East Sale for a precautionary landing.  After touchdown and throttling back the stbd engine, the engine surged and caused the aircraft to slew violently to port and skid uncontrollably on the wet runway and crash into a tree at the side of the runway.  Damage was extensive with both airscrews and port mainplane written off.  Pilot 410166 PLTOFF S. Matthews, Navigator 410912 PLTOFF SL Anderson, WAG 417175 PLTOFF RA Graetz and WAG 410995 SGT F. Maloney were uninjured.

Accident at Werribee Satellite Airfield.  On take off Trainee 438796 LAC A.M. Haylock was under the blind flying hood when, at 100ft, the port engine failed.  The Instructor 401805 FLGOFF K.M.A. Smith immediately took control of the aircraft, which was rapidly losing height. The instructor had the option to go-around against the yaw of the dead engine while crossing over Werribee township.  Deciding against this risk he decided to land straight ahead on Werribee racecourse, resulting in 100% damage to both airscrews; 100% to both brakes; 5% to front fuselage and nose section; and, 5% to centre fuselage section.  Repairable at unit.  Pilot FLGOFF K.M.A. Smith and Trainees 432365 LAC G. O'Connor & LAC A.M. Haylock uninjured.

Sold by DAP at the Aircraft Remnants Auction RAAF Mallala to Mr James Woods of Maitland, SA. The wing spar was used to make a glider and the remainder was sold to Mr. Bubner of Port Wakefield, SA.  By 1967 all that could be located at Bubner's farm was a set of cowlings. This was the first Oxford towed out of RAAF Mallala after the Auction.  Significant road traffic problems were caused by the Mallala sales aircraft being towed, with their wheelbase barely fitting on the main roads of the time, forcing all other vehicles off the road. Police escorts were required from Gepps Crossing for those aircraft being towed through the Adelaide suburbs.

Poor airmanship when solo low flying caused the aircraft to crash land into the side of a hill at 1005 hrs near Meriton, SA.  Estimated damage was: Fwd fuselage 75%; centre fuselage 45%; undercarriage 35%; stbd airscrew 95%; and, various other areas damaged.  The pilot FSGT W.A. Cruickshank was slightly injured.

X7139 took -off from Werribee Satellite Airfield at 0745 hrs in a northerly direction. The sole eyewitness stated that as the aircraft climbed to about 200ft it collided with Oxford R6194 and X7139 then immediately dived into the ground where it burst into flames and incinerated the two crew members. R6194 also lost height rapidly crashing through HT lines and trees on the NE boundary of the airfield before coming to rest in a paddock on the west side of Geelong Road.  R6194 was also completely destroyed in the accident; two of the three crewmen aboard were killed and 432525 LAC J.G. Legoe was seriously injured. Those killed in the accident were:

R6194 Pilot 271521 FLTLT George Hector Howard (40) married of Childers, QLD and Trainee 20755 LAC Arthur Austin Hughes (32) married of Richmond, VIC

FLGOFF Allen was cremated at the Springvale Crematorium, Melbourne and is interred in Wall 2 Sec. AA Niche 519

LAC Lynch is buried in the Springvale War Cemetery, Melbourne. Grave 2.S.D.11

FLTLT Howard is buried in the Springvale War Cemetery, Melbourne. Grave 2.S.D.7

LAC Hughes is buried in the Geelong Eastern Cemetery Plot D. Row A Grave 5

When conducting a three hour, long distance cross-country exercise the aircraft encountered bad weather near Burra, SA.  The weather became so bad the pilot elected to make a forced landing at Farrell's Flat but on touch down the aircraft swung violently to stbd.  The pilot saw a large creek bed rapidly approaching and decided to retract the undercarriage. His actions however were too late and the aircraft dropped into the gulley and suffered considerable damage. Airframe 25% damage; port undercarriage 50%; stbd undercarriage 75%; port wing 98%; stbd wing 90%.  Repaired at unit.  Pilot 410768 LAC W.D. Young and Navigator 20442 LAC D.I. Robertson both suffered minor injuries.

Trainee solo pilot 439852 LAC F.L. Gardner performed a forced landing following engine failure at 0825 hrs.  The aircraft bounced badly and crashed into  a nearby railway embankment where it burst into flames.  Aircraft a complete write-off but the pilot managed to escape and suffered only minor injuries.
