Pratt and Whitney Twin Wasp S3C4-G#30382 and #31402. 29/05/44 Rec 15ARD RP(Res Pool) ex 5AD. 05/06/44 Pilot on ferry from 15ARD to Nadzab found that the pitch levers would not operate airscrew in either automatic or manually operated position. On approach to land at Nadzab Strip, engines at 900RPM,  airspeed at 100 knots, it appeared to him he was overshooting and lowered his flaps, before the undercarriage was locked. In an attempt to go around again, Pilot had failed to allow sufficient time for undercarriage to come down and to lock, thus undercarriage collapsed and the a/c crashed landed. A/c swung violently to starboard, but was corrected. Thinking that the power on the starboard engine had cut, he eased off power of the port engine to keep the a/c straight. By the time the pilot realised that the starboard tyre had been punctured, the a/c swung off the strip into the sand, causing the starboard then the port undercarriage collapsed. The a/c then skidded across the sand coming to rest at the end of the strip. Fitted with Dual Controls. 04/12/44 Rec 22 Sqn RAAF ex 5AD. 17/10/45 On ferry trip from Morotai to Higgins Field then to Forest Hill, when a/c was requested to divert to Daru Island after landing at Higgins Field to air evacuate a desperately ill woman on the request of the Senior medical Officer of the Army Hospital at Thursday Island. After making enquiries as to the suitability of the emergency landing airstrip (880yards long) on Daru Island, he felt he could land there. On landing successfully, (1645Hrs) the pilot felt the airstrip was much shorter than expected. The ill Woman turned out to be a "Samoan" girl of 6 years of age.   However, on take-off, a/c hit hidden stump on strip at 55-60 knots, causing it to swing portside off the strip into a ditch, then sheering off both undercarriage legs before stopping and facing 270º to the take-off direction. 5AD Ferry Crew: Pilot: F/Lt Craig Kirkpatrick Serv#428326, F/O Geoffrey Harold Roper Serv#443799, F/Sgt P R Leunic, Cpl R A Howard and LAC A Leavers. But on this diversion flight, the crew was limited to two: Pilot: F/Lt Craig Kirkpatrick Serv#428326 and F/O Geoffrey Harold Roper Serv#443799, and on take-off from Daru Island, additionally with father of the 6yro girl, the 6yro girl and the Daru Island Medical Officer, Lt R Penny. GR/B. 18/04/44 Allotted 5AD Storage ex 1AD. 21/05/44 Accident at Laverton Aerodrome when a/c had returned from Parafield after transporting a Test Pilot there to ferry RNEIAF Lodestar LT932 to Laverton, when on being taxied after landing when tail strut collapsed. Rivet mounting the tail strut had sheered. Pilot realised he was overshooting the short strip but was too late to go initiate a go around and passed the end of the strip, starboard undercarriage collapsed and crashed into trees on the side of the strip. GR/B.  16/02/44 Allotted 1APU(Air Performance Unit) for prototype installation of long range radar. 29/03/44 Rec 1APU ex 1AD. 23/08/44 Rec 5AD Storage ex 1APU. 27/09/44 Rec 5AD ex 5AD Storage. 25/01/45 Rec 32 Sqn RAAF ex 5AD. 04/04/45 when a/c was engaged on a travel flight as authorised by AOR Eastern Area to Leyte, PI. when on return to Kamiri Strip, Noemfoor Island DNG, it was found that the a/c experienced some hydraulic sluggishness, requiring the use of the hand pump to lower the undercarriage. On the ground the hydraulic system was found to have air in it. It was drained and refilled., ready for an air test the next day. 05/04/45 Accident when following its take-off, it was found that the undercarriage failed to fully retract. All measures to lower the undercarriage failed, resulting in a crash landing after all the fuel in the fuel in the wing tanks had been jettisoned prior. No AAA was observed. When the a/c was on or near of its bomb release point, there was a large flash of flame and smoke, with the a/c completely disintegrating into very small pieces. Blast effect was felt by the other a/c near, and it was believed that all bombs exploded either simultaneously or that one bomb in the bomb bay had exploded prematurely and detonated the others. (Three days earlier A9-650 had exploded in an identical way during a bomb run over the Maprik area. It was believed that all bombs exploded either simultaneously or that one bomb had exploded prematurely and detonated the others. A9-627 continued to fly for a few minutes before going into a shallow dive whereupon it hit tree tops some distance from the other a/c crash site, and caught fire and exploded after the impact. At 1905hrs a recall was sent which was received and acknowledge by A9-630 at 1923hrs. A radar plot from 109FCU (Fighter Control Unit) at 2130hrs showed the a/c (IFF Code 1)45 miles from base heading for Dagua Strip, which when told, advise by radio to their a/c in plain language to "Steer 260º base range 65 miles " to go to Dagua Strip" at 2040hrs. This A9-630 acknowledged at 2045hrs. Reported sightings from 17th AIF Brigade unit located Southeast of Cape Djeruan between 2030 and 2115hrs at a low altitude with all lights showing heading east, then soon after, heading west. A/c posted as missing. It was believed that all bombs exploded either simultaneously or that one bomb in the bomb bay had exploded prematurely and detonated the others. Approximately after 20 minutes of flight, the pilot smelt burning rubber and decided to make for the closest strip, being Boram Strip. Efforts to contact by wireless Tadji Tower or Tadji Aeradro was unsuccessful. On approach it was noticed that smoke had started to enter the cabin. It was thought the fuse in the wireless may have been the cause and was shut off. After touching down it was noticed that the starboard engine was on fire. On an attack on Karadia Village ,  four bombs were released in salvo resulting in the a/c  being damaged by shrapnel from its own 40lb fragmentation bombs explosions released at 100ft instead of the safe minium parameter height of 500ft.  The a/c received numerous holes from shrapnel fragments, from pressure line damage to joining the two port oleo legs severed, damage to rear spar, and numerous skin punctures, and worryingly, to both port and starboard main fuel tanks which despite having been self-sealing, could not keep up. This resulted in the immediate return to Tadji Strip.
