Coded FX-P. 1610hrs 04/10/43 Turnbull Strip, Milne Bay Papua,   owing to hydraulic failure, the undercarriage was pumped down by the emergency system as was the flaps at 15º angle. On landing the aircraft brakes seem ineffectual, and so the engines were stopped, then the a/c would not pull up, and with brakes "on', rolled off the end of the strip into undergrowth. Coded SK-A Overscored. 1015hrs 19/08/45 on taking off from Labuan Strip, Borneo, ran straight up to 70 knots when either by soft ground or binding brakes, the a/c a violet swing to port. The throttles were fully open and the a/c lifted off the runway with the pilot correcting the swing of 25 yards to port, when the a/c hit a five-foot mound of dirt then came to a sudden rest a few feet on. NFDs.  28/01/44 Rec 26RSU ex 6 Sqn RAAF. 11/02/44 Rec 6 Sqn RAAF ex 26RSU. 02/03/44 Operational loss after taking off 0605hrs from Vivigani Strip to complete a SGX patrol in the vicinity of China Straits, south of Milne Bay entrance. 1020hrs starboard engine failed in flight at 1100 feet altitude. Unable to maintain flight on one engine, the bombs could not be jettisoned, and with the a/c losing more height, the a/c was then ditched 10 miles south of China Straits. The a/c floated for 10 seconds, enough time to inflate the rubber dinghy, with the crew eventually being rescued by the Royal Australian Navy River Class Frigate, HMAS Gascoyne, one hour later. Pratt and Whitney Twin Wasp S3C4-G#56520 and #55217. 15/09/43 Rec 1OTU ex 1AD. 0530hrs 07/10/43 Whilst on a night exercise (DR10(T)) from East Sale, the a/c was seen to crash near Yarram Vic. Later amended as crashing into the sea in 10 fathoms of water just 1.5miles east of Woodside Beach VIC, after one body (Sgt Sexton who was later buried at the Sale War Cemetery, VIC) and parts of the wreckage* were wash ashore. Search for the rest of the crew was abandoned.   TTHRs 70.15hrs. Dual Control.  Pratt and Whitney Twin Wasp S3C4-G#56657 and #56512. 15/08/43 Rec 5OTU ex 2AD. 1300hrs 05/11/43 Accident when 60 miles from Bankstown NSW on a travel flight from RAAF Tocumwal. when the a/c started to pitch fore and aft, then endeavoured to bunt two to three times. by pulling off power and exerting considerable back pressure on dual controls with the aid of the Observer, control was regained.  and the a/c was flown then landed flaps retracted and wheels up (due to becoming extremely nose heavy) at Bankstown Aerodrome. TTHRs 184.50hrs. Issue and cause was a defective elevator trim tab*. A9-417 MkVIII  30/08/43 Rec 1AD ex DAP. 05/10/43 Rec 1OTU ex 1AD. 0100hrs 25/04/44 a/c took off on a training exercise (DR10)  to seaward: East Sale/Paynesville/Blighpoint/Currie/Hogan Group/Yarram/East Sale. 0345hrs a/c crashed into the sea, in 15 fathoms of water, some 5 miles  to seaward, position 37º 57' South/147º 50' East between Lakes Entrance and Metung, Vic. Rubber dinghy with equipment intact, pieces of a fuel tank, one landing wheel and part of undercarriage strut were recovered. Damage indicated a highspeed impact. TTHRs 243.30Hrs. Pratt and Whitney Twin Wasp S3C4-G#56593 and #56612. 26/09/43 Rec 1OTU ex 2AD. 0830hrs 05/03/44 after taking off from East Sale Aerodrome, the a/c was involved in authorised low flying fighter affiliation, when during an evasion, struck the water on Lake Bunga, near Lakes Entrance, with the port airscrew, resulting in the loss of the airscrew, together with the reduction gearing and a fire which was extinguished. A/c performed an emergency landing at Bairnsdale Vic. TTHRs 229.50Hrs. A9-423 MkVIII 29/08/43 Rec 2AD ex DAP. GR/B. Pratt and Whitney Twin Wasp S3C4-G#56635 and #56656. 22/09/43 Rec 1OTU ex 2AD.    0955hrs 05/11/44 after taking off from East Sale Aerodrome, the a/c was involved in authorised low flying instrument and low flying, when during a turn struck the water at Lake King, 20 miles east of Bairnsdale VIC, with one airscrew. The airscrew was bent backwards, fracturing the reduction gear housing which on landing detached from engine and struck the tailplane. They managed to land the a/c. Crew: Pilot: F/Sgt Thomas Charles Terrill Serv#409768 and Student Pilot: F/Sgt Roy Mansfield Wishart Serv#417143 were not injured. With the engine throttled back, the engine caught fire, the airscrew feathered and then the fire was extinguished. On final approach, Wewak PNG, 1120hrs (north to south Strip) a grader attempted to cross the runway causing the pilot to throttle port engine to clear the grader (belonging to an Army Engineering Unit) and then continued to sink and landing much further down, then crashing off the end of the runway, pulling right to avoid a gully. Coded UV-S.  0650hrs 25/12/43 a/c was returning to Vivigani Strip after carrying out a 10 a/c attack on a target in the Rabaul New Britain Area. After a normal circuit, the a/c touched down at 0655hrs on the bomber strip from 10-20 feet on mains, after being held a little too high. On reaching the zenith of 20 feet, a heavy explosion occurred slightly to the starboard and just behind the mainplane of the aircraft, causing the tail of the a/c to rise in the air, then the whole fuselage rear of the mainplane and the whole tail of the aircraft disintegrated. The centre section, wings and nose section continued through the air for approximately 200 yards after the explosion had taken place. The fuel tanks in both wings then denotated, with the remaining sections striking the ground before resting, then exploding and burning fiercely. Result was caused by a non-jettisoned/hung bomb that became loose/or dropped following bounce. The Ambulance and fire tender were on the scene within seconds, but all crew members were killed before any assistances could be given. Coded KT-K. 15/01/44 Accident when carrying out a anti-submarine patrol to seaward of Cairns, Queensland at 1000 feet, when the pilot manipulated the fuel tank levers to operate one tank in order to equalise the contents of both tanks. For a few minutes, everything was satisfactory until the starboard engine failed. The pilot trimmed the a/c for single engine operations, and turned the other tank on, which had no effect and then the port engine failed. Being low by then, advised all crew for ditching and was jettisoning the bombs and fuel from the wing tanks, the a/c then immediately dropped 10 feet and  hit the water with the bomb bay doors open into the sea 2.5 miles away from a RAN Corvette, and about 50 miles east of Cairns. The A/c sank immediately. Crew: Pilot: F/Lt John Edward Dures Serv#290794 Lacerated mouth and shock, and WAG F/O Arthur Joseph Prentice Serv#411257 suffering a slightly injured shoulder disc, injured right elbow and also with shock* , managed to enter into a life raft. Both were rescued by  a whaler from the RAN Corvette(HMAS Bowen) twelve minutes after.
