Accident when aircraft after suffering engine failure, force landed 30/10/46 in three feet of water on beach at Nelsons Bay NSW during a training flight. Pilot, F/O L J Hart Serv#419747 injured. A20-404 CA-8 606 Rec 1AD ex CAC 29/10/41. Rec OTU Nhill 13/12/41. Accident 1330 hrs 12/02/42 near Horsham when aircraft collided with A20-325 in air damaging fin, stern post and rudder. Pilot, P/O D R Baker, made an emergency landing in field. Pilot of A20-325, Sgt W M Booth returned to base with damage to starboard spar, leading edge and bent wing. Repaired. Accident 14/03/42when aircraft crashed into Lake Glenmaggie North of Heyfield Victoria Australia. Crew; SGT Noel Edmund Giddings 416021 (Pilot) who was killed and F/Lt Dawson (Intelligence Officer) injured. SOC 26/03/42 with aircraft insitu. Lake Glenmaggie was dry in 1968 when the engine was recovered and again in 1982 when the Australian Aircraft Restoration Group recovered the port wing from the lake. The next day it was dumped at the Moorabbin tip. The wheels were recovered and form part of a memorial to the crew at the Lake Glenmaggie Dam wall. A majority of this aircraft is being held in storage for long term restoration.

F/Lt Gil Cory (pilot) and Flying Officer Bill Tucker of 5 Sqn RAAF loaded with 3 Storepedoes on Xmas Re-supply trip from Piva North Strip (take-off 1045hrs)to the Army suffered a drop in Boost (down to 15 inches of boost) when flying up a gully and crash landed, tearing off both wings on a 60ft x 30ft ledge on a Mountain by stall landing. Both received minor injuries (4 broken ribs for Cory). Natives helped the crew and received a note from Paul Mason (AIB) to stay put. A Cpl White (AIB)and two Police boys escorted them to a village on the 30/12/44. A Wirraway from 5 Sqn RAAF dropped supplies by way of storpedoes on the 05/01/45. They trekked back to 2/8th Cav Commando on the 06/01/45. SOC 18/01/45.

Accident 1350 hrs 04/04/45 near Wagga Rd Saudy Creek when during formation flying, A20-391 collided with A20-415. Pilot of A20-391 Sgt M P McGee Serv#442296 killed and Pilot of A20-415 Sgt Mc Dowell Serv#438604 bailed out and survived. Aircraft crashed and was totally destroyed by fire. Accident 0950hrs 23/04/43 the aircraft was involved in a mid-air collision during a formation break with A20-442 some 3 miles SW of Uberta Post Office near Uranquinty. Pilot, FLT K R Milne and Trainee LAC S J Bromhead Serv#33011 made a forced landing without injury. Crew of A20-442, F/Sgt J F Ward Serv#406086 and LAC I S Parker Serv#429480 both baled out of stricken aircraft without injury. Repaired and in service 03/05/43. The crew of A20-131 baled out, though one crew member (LAC Rayner) was killed when his parachute opened too late. The RAAF School of Army Co-operation was set up to train "Pilot" Officers and NCOs (with a maximum of 15 students per class) on the art of Air to ground support/ Reconnaissance/ Message dropping and basic map reading in support of the Australian Army Ground forces. It was also to train Army personnel in the reverse manner. Basically to be trained by Australian Army and Air Officers and senior NCOs in identifying prominent cartographic features in grid maps from the air in the first instance to know where they were, then followed by lessons in identifying troops, positions and vehicles during aerial searches and to report accurately their findings in standard formats. Additionally, to train Pilots to set their aircraft up correctly so that a rear crew member (Wireless/Air Gunner/ Observer) can use hand held cameras for photographic reconnaissance. From there they would go onto 4 and 5 Squadron (Army Co-op) RAAF who were situated at Canberra and Laverton, respectively in this period.

Following a successful combat mission leading in RNZAF Corsairs on the 06/07/45, the aircraft on returning to Pivoe Strip Bougainville commenced a right turn which saw the starboard wing hit the water, causing the aircraft to crash. Crew F/O Brian O'Connell Serv#437192 and Navigator P/O John Richard Hatfield Serv#25350. Eye witness report seeing the aircraft strike the sea, with crew getting out of aircraft and attempting a boarding a life raft that Hatfield had deployed. O'Connell, his head covered in blood and dazed from striking his head on impact, was unable to get into the life raft, despite Hatfield trying to bring him aboard. O'Connell disappeared underneath the raft and drowned. Hatfield himself also suffering a fractured skull, was rescued by AIF troops after paddling to shore and was the conveyed to MDS. He was later evacuated to Australia by air to 2/1 Australian General Hospital. Airframe approved for write-off and was SOC 16/07/45 per AMSE and issued 10 RSU 06/08/45 ex 5Sqn RAAF.

Ground Accident Canberra 05/03/42 when aircraft on completion of flight suffered fire damage. Engine was running slow, however flooded carburettor caused flames from exhaust and started fire, causing damage on starboard fabric panel. Rear occupant of aircraft Sgt B H Smith Ser#8603, suffered burnt forearm. Pilot; P/OR H Drabsch not injured. Repaired by Squadron. Between 12RSU and 4Sqn for services over next year. Coded QE-V. Ground accident, 26/02/44, when taxying aircraft from dispersal at Gusap. Aircraft hit fuel dolly causing damage to airscrew. Pilot was F/O J Ahern Ser#426817. Repaired. Aircraft missing 15/04/44 after taking off at 07.55hrs on a operational tactical reconnaissance flight over the Wantoat area from Gusap Strip, New Guinea. In December 98 the aircraft was discovered and a team of RAAF members traveled to the site of the crash. After talking with local villagers it was found that one villager who, only still a boy, had trekked to the site with other villagers and discovered the aircraft and it's dead crew. The crew were then buried next to the aircraft on the hill where it crashed. After some interviewing of some villagers it was found that the pilot reportedly crashed while being chased by a Japanese A6M ""Zero"". The crew were buried with full honours at Lae War Cemetery on 13 January 1999. Pilot of A20-499, Sgt D Scott. Pilot of A20-506 was Sgt J J Willis. Both not injured. Accident 1640hrs 14/10/42, whilst on exercise, dived and crashed into Lake Victoria in New South Wales. Pilot, F/Sgt Lloyd George Wellington Serv# 407472, killed. Airframe salvaged on 27/10/42 from Lake. SOC 04/12/42.

Approval to dispose of from Storage at Point Cook 06/05/58. Sold to CAC 02/05/58 for conversion to a CA28 Ceres. SOC 03/07/58. Sales Advice SV.40232, cost 750 pounds.
